Connecting housing clients with needed resources, and working with existing housing programs and community partners to leverage additional resources and opportunities to help clients achieve stable housing and self-sufficiency. This is a high-impact position that requires strong interpersonal skills and the ability to communicate effectively with a wide variety of stakeholders.Essential Functions* Supports clients to overcome housing barriers and achieve permanent housing placement* Creates and maintain a community resource list* Completes initial evaluations, determine eligibility, and engage participants in individualized housing stability plans with follow up case management* Assists clients in achieving financial literacy by setting up bank accounts, creating household budgets, and helping them identify strategies to pay bills on time* Assists clients by utilizing warm transfers and referrals to employment and education services* Provides elder/disabled clients with resources and support to Age in place," which may include applying for SNAP benefits, SSI/SSDI, VA benefits, Medicaid/Medicare, etc* Attends training to ensure that the program remains in compliance with the grants stipulations* Ensures compliance with Tribal, federal, state, and local employment laws and regulations, Tribal policies and TERO ordinanceIt is the intent of this job description to highlight the essential functions of the position; however, it is not an all-inclusive list.
