Location: Anchorage, AK
Contract Type: Travel Contract
Shift Type: Monday-Friday (5x8 days), one of the following day shift windows: 7:00 AM - 3:30 PM OR 7:30 AM - 4:00 PM OR 8:00 AM - 4:30 PM OR 8:30 AM - 5:00 PM (Day shift). Weekends: required every other weekend (or 4 weekend days in a 4-week schedule).
Hours per Week: 40
Candidate Pay Rate: $74 per hour

Roles & Responsibilities
- Coordinate comprehensive case management and utilization review to ensure patients receive the right care at the right time
- Lead discharge planning and needs assessments, ordering DME and arranging safe transitions home or to other levels of care
- Develop, document, and follow individualized plans of care using Epic and Milliman guidelines
- Serve as the liaison between patients, families, clinical teams, and community resources to streamline care
- Perform timely Epic charting and documentation in compliance with CMS and HIPAA regulations
- Participate in interdisciplinary rounds and leverage support services (PT, pharmacy, social work, etc.) to optimize outcomes
- Float within the case management team as needed to cover service gaps while maintaining scope of practice
- Provide patient education and advocate for discharge needs to reduce readmissions and improve patient satisfaction
Qualifications
- Active AK RN license in hand required for assignment/auto-offer (no pending-only for auto-offer)
- Minimum 1 year acute care nursing experience (first-time travelers accepted)
- Epic experience required (Epic hardstop expectation β charting competency expected)
- Acute hospital case management/utilization review experience required
- ACM certification preferred (ACM-RN is a plus)
- BLS preferred (not strictly required)
- Ability to work required weekend coverage and float within case management services
- Candidates must be non-local (permanent address at least 50 miles from facility)
- Comfort with regulatory requirements (CMS/HIPAA) and utilization guideline application
Knowledge & Skills
- Case management and utilization review workflows
- Care coordination and discharge planning best practices
- Needs assessment and ordering DME
- Using Milliman guidelines for utilization decisions
- Proficient Epic charting and electronic documentation
- Familiarity with Microsoft Office and Teams for communication and reporting
- Comfort supporting diverse patient populations: neonates through geriatrics, pediatrics and women's services exposure
- Strong interdisciplinary communication and patient advocacy
