Position Overview: Healthcare Outreach Representative
Sigma Systems Inc. is currently seeking a Healthcare Outreach Representative for one of our clients based in Pittsburgh, PA. This role is ideal for individuals passionate about community engagement and preventive healthcare. The position involves extensive fieldwork, collaboration with community partners, and direct interaction with members to improve healthcare access and outcomes.

Note: Due to the governmental nature of this position, only U.S. Citizens are eligible to apply.

Work Schedule:

  • Remote position to start; candidates must be local to Allegheny, Westmoreland, or Beaver counties
  • Field travel required over 50% of the time (travel reimbursable)
  • Monday to Friday schedule
  • Weekend work required once per quarter

Key Responsibilities:
  1. Community Outreach & Engagement:
    o Initiate face-to-face outreach efforts related to preventive health, maternity care, care transitions, and chronic condition initiatives
    o Develop rapport with members and community centers to foster supportive relationships and promote active engagement in health and wellness
    o Represent the organization at community events, including those sponsored by community centers
  2. Member Education & Support:
    o Educate members on the importance of preventive health services and available benefits
    o Assist members in completing Health Risk Assessments and provide direction in managing healthcare barriers (e.g., transportation, financial, knowledge gaps)
    o Support appointment scheduling and address member inquiries
  3. Care Coordination & Referrals:
    o Assess opportunities for care management intervention and make appropriate referrals, including to Behavioral Health Managed Care Organizations (BHMCO)
    o Coordinate with members, providers, and community agencies to ensure access to preventive health services
  4. Collaboration with Community & Agencies:
    o Work with human services providers such as Head Start Programs, WIC, community centers, and homeless shelters
    o Collaborate with state agencies including Children and Youth Services (CYS) and juvenile probation
    o Identify and outreach to members in need of services
  5. Training & Program Development:
    o Contribute to the development and preparation of educational materials for members and providers
    o Conduct training sessions on Early and Periodic Screening, Diagnostic and Treatment (EPSDT) services and other preventive health initiatives
    o Provide summaries and outcomes of training to management and peers
  6. Continuous Improvement & Reporting:
    o Participate in developing effective, measurable, and innovative outreach programs, especially for at-risk populations
    o Understand the organization's lines of business and member benefits to ensure accurate communication within the community

Qualifications:

Education:
o High School Diploma or GED required

Required Experience & Skills:
o Medical Assistant experience required
o Call center experience required
o Strong knowledge of customer service principles, including needs assessment and service quality standards
o Ability to work collaboratively with peers, supervisors, and customers
o Strong problem-solving skills with a proactive, self-directed, and creative approach
o Ability to manage multiple tasks simultaneously and respond promptly to customer needs
o Excellent written and verbal communication skills
o Cultural competency and sensitivity toward Medicaid populations, including those with limited English proficiency
o Understanding of healthcare barriers such as transportation, childcare, financial challenges, and distrust of the healthcare system
o Demonstrated patience, empathy, and professionalism in all interactions

Certifications & Requirements:
o Act 33/34 and 73 clearances required (not required at submission; must be obtained prior to start if selected)

Preferred Qualifications:
o Experience in healthcare-related customer service or marketing environments
o Bilingual (Spanish-speaking preferred)
o Basic knowledge of medical terminology
o Experience working with targeted communities or populations
