Responsibilities

As a Transitional Care Specialist in Hospice, you will:

  • Act as the representative for the branch in transitional care initiatives and engage professionally with patients, families, physicians, hospitals, senior living communities, and other healthcare organizations to promote Enhabit Home Health & Hospice services.
  • Create patient-centered transition plans that improve quality and efficiency by incorporating clinical guidelines and protocols.
  • Serve as a community ambassador, educating the public on the services we provide.

Qualifications

To be considered for this role, you should have:

  • A degree from an accredited program in nursing, therapy, or social work.
  • A current state license to practice.
  • A minimum of one year of experience in home care.
  • A solid understanding of federal, state, and local regulations regarding hospice operations.
  • Basic technological proficiency, including mobile device usage.

It is preferred that you have:

  • Experience and knowledge in hospice care.

Requirements

You should also:

  • Possess a valid state driver's license.
  • Maintain automobile liability insurance as required by law.
  • Have reliable transportation that is in good working condition.
  • Be able to drive safely in all weather conditions.
