POSITION SUMMARY

Provides assistance to patients from the Maniilaq Service Area traveling to Anchorage for medical appointments or procedures. Services provided include translating, transportation, emotional support and assistance accessing funds when appropriate, may assist with providing Inupiaq translation, discharge planning, long-term care placement, assisted home living placement. Hours required need to be flexible, and may include evenings and weekends in order to meet the needs of clients. Willing to travel to Kotzebue as needed to cover patient advocacy office as needed.

PRINCIPAL DUTIES AND RESPONSIBILITIES

    Provides support for patients and families requiring services at ANMC and other providers in Anchorage. Provides transportation services to and from Anchorage International Airport and to housing and other providers and services in the Anchorage area. Provides Inupiaq translation services at ANMC. Assists families with coroner, mortuary and burial services, funeral home coordination, transportation of body etc. Works in coordination with patience experience department at ANMC, Maniilaq patient advocacy and case management departments.
  1. Demonstrates knowledge of and maintains skills necessary to provide care appropriate to the age of the patient being served.
  2. Participates in Continuous Process Improvement projects. Follows the Maniilaq Travel Policy guidelines documents and distributes per-diem cards to patients and their escort. Provides support for ANMC patients and families with discharge needs including durable medical equipment.
Job Qualifications: MINIMUM REQUIREMENTS

High school diploma or equivalent or one-year experience in health care setting. Must have a valid driver's license with a good enough driving history to be covered by Maniilaq Association insurance. Ability to communicate with patients, families, Medical Center staff and referral agencies (such as hospitals, nursing homes, pharmacies, mortuaries, etc.)
