The Clinical Care Facilitator plays a vital role within the multidisciplinary care team at PMGA. This position is focused on coordinating services for patients, particularly those facing challenges in navigating the healthcare landscape. Key responsibilities include managing specialty referrals, assisting patients in self-care, and connecting them with relevant community resources. The Clinical Care Facilitator will also collaborate with external service providers to ensure seamless transitions of care and provide comprehensive patient education to enhance access to essential resources.
