The Clinical Care Facilitator plays a vital role within the multidisciplinary care team at PMGA. This position is focused on coordinating services for patients, particularly those facing challenges in navigating the healthcare landscape. Key responsibilities include managing specialty referrals, assisting patients in self-care, and connecting them with relevant community resources. The Clinical Care Facilitator will also collaborate with external service providers to ensure seamless transitions of care and provide comprehensive patient education to enhance access to essential resources.

Required Qualifications:

• Associate's Degree in a Healthcare-related field, or 2 years of experience in an outpatient setting in lieu of an Associate's degree, along with related coursework/training.

Preferred Qualifications:

• Bachelor's Degree in a Healthcare-related field.
• 2 years of experience in care coordination.
• Experience in an outpatient setting.

Compensation will be determined based on relevant experience and other factors. Additional compensation may be available for this role, including shift differentials or bonus opportunities.
