Guardianship Case Manager

The Guardianship Case Manager is responsible for the day-to-day management of client cases in guardianship status with Tribal Court. This includes coordination with Case Workers, development and monitoring of service plans to ensure the consistent provision of quality services in alignment with client-directed goals and objectives, and facilitation of internal and external resource referrals for legal guardians appointed by the Tribal or other Court Systems.

Essential Functions:

  • Provide resources to increase access for social, educational, career, housing, and other supportive services on behalf of guardians legally appointed to care for the person/wellbeing of children and/or vulnerable adults.
  • Appear in Court proceedings on behalf of guardians.
  • Conduct case management to ensure compliance with electronic case management systems.
  • Complete client's assessment and report to the Courts, Social Security, Medicaid, and other authorities.
  • Monitor and prioritize workload to ensure deadlines are met.
  • Provide training for guardians in parenting skills and child safety.
  • Facilitate access to resources and application processes for children and families.
  • Consistently and effectively utilize program procedures, meet grant requirements and state and federal regulations.

Working Conditions: Lifting Requirements - Light Work: Exerting up to 20 pounds of force frequently, and/or a negligible amount of force constantly to move objects. If the use of arm and/or leg control requires exertion of forces greater than that of sedentary work and if the worker sits most of the time, the job is considered light work.
