The Clinical Care Coordinator participates as an integral member of the multidisciplinary care team at PMGA. Key roles are coordination of services for panel patients, focusing on those who face particular difficulty with navigating the healthcare system, management of specialty referrals and supports patients in self-care and identifying relevant community resources.
