Job Responsibilities

The Southcentral Foundation (SCF) Case Management Support is responsible for coordinating and managing administrative duties for a Primary Care Provider (PCP) team and the wider Integrated Care Team (ICT). The CMS works closely with the core medical team as a primary contact between the customer-owners and ICT, ensuring effective schedule coordination, communication, information transfer, and system coordination.

This position has two levels, designed to provide progressively more responsible and independent work experiences. Progression between levels depends on demonstrated ability to handle increased responsibilities.

Qualifications

SCF programs serve primarily Alaska Native people affiliated with Cook Inlet Region, Inc. (CIRI), and Alaska Native and American Indian people within SCF's geographical area. Employees should have a thorough understanding of the cultures and needs of this population to support SCF's vision of wellness and its mission of working with the Native Community to achieve health and related services.

  • Bachelor's degree or four years of administrative support experience, or an equivalent combination of education and experience, or demonstrated proficiency as an Administrative Support III at SCF.

Additional Qualifications for Case Management Support II

  • Two years of Case Management Support experience, or demonstrated proficiency as a Case Management Support I at SCF.

Native Preference

Under P.L. 93-638, as amended, SCF pursues a policy of Native preference in hiring, contracting, and training. Certification from SCF Human Resources is required before applicants receive preference.

Employee Health Requirements

Compliance with SCF's Employee Health Procedure is mandatory. All job-related health screenings and immunizations must be completed prior to employment. For healthcare positions, documentation of immunizations such as MMR, Varicella, Hepatitis B, Influenza, T-dap, and COVID-19 is required.
