Job Title: Case Manager I/II

General Functions:

As an integral part of the multidisciplinary treatment team, the Case Manager plays a key role in supporting participants throughout their recovery journey. Case Managers deliver comprehensive case management services across Recovery Services' Continuum of Care, including both outpatient and residential programs. They link participants to appropriate services, facilitate skill-building in both community and office settings, coordinate care, and provide psychoeducation-all guided by each participant's individualized treatment plan. Using a strengths-based, culturally responsive, and trauma-informed approach, Case Managers empower participants to work toward their treatment goals and prepare for long-term success beyond treatment. They actively engage clients, build meaningful rapport, and are skilled in crisis intervention, life skills training, and group facilitation.

Duties and Responsibilities:

Case Manager, I

• Efficiently manage the case manager's schedule and caseload to ensure at least 75% of total work hours are dedicated to direct-service related activities, to include: a minimum of 50% of total work hours spent delivering billable services. Up to 25% of direct service-related time may be allocated to non-billable participants support, such as external care coordination (without the participant present), transportation, outreach, engagement, and case consultations.
• Create and manage clinical documentation for program participants including assessments, diagnoses, treatment plans, case staffing, discharge summaries, and progress notes
• Ensure timely and accurate completion of participant records within the electronic health record system, including reports, individual and group session notes, assessments, and related documentation.
• Maintain collaborative relationships with partnering agencies and referral sources.
• Coordinate care with referral sources, family members, community agencies, and related documentation.

• Assist in the coordination of participant placement/transfer into treatment levels beyond assigned programs.
• Follow-up on referrals as directed by clinical leadership.
• Communicate updates on case management goals, objectives and participant progress with other treatment team members.
• Participate in and observe individual and group sessions to support participant treatment plans.
• Support the administration of Urine Analysis (UA) and Breath Alcohol Concentration (BrAC) testing for participants.
• Attend required training, support meetings, and staff development training.
• Collaborate with team members in regular staff meetings and case-staffing.
• Coordinate and provide the transportation of participants to activities, case management services, and program-related outings.
• Deliver individual and group case management, skills training, and psychoeducation sessions focused on substance use and recovery goals, based on participant needs and treatment plans.
• Foster and maintain respectful, professional relationships with participants, alumni, family members, and peers.
• Represent the program in community meetings and events as needed.
• Support participants in self-administration of prescribed medication as directed by medical providers.
• Coordinate with medical providers regarding participant care, as appropriate (residential programs only).
• Maintain confidentiality and demonstrate a professional demeanor at all times.
• Maintain all credentials in a current and up-to-date status.
• Perform other duties to support program goals and operations as needed and assigned.

Case Manager, II

• Perform all duties of a Case Manager I with proficiency.
• Provide support to Case Manager I staff by offering guidance, answering questions, and making recommendations regarding participant care.
• Demonstrated working knowledge of ASAM and DSM-5 diagnostic tools.
• Utilize ASAM, approved screeners, and DSM-5 criteria, to conduct substance use assessments, make substance use diagnoses and recommendations for treatment, when assigned.
• Demonstrated working knowledge of applicable state and federal regulations governing service delivery.
• Demonstrated working knowledge of community resources available to support participant needs.

Job Responsibilities Related to Participant Privacy

• Employee is expected to actively participate in CITC privacy training and to protect the privacy of participant information in accordance with CITC's privacy policies, procedures and practices, as required by federal and state law. Failure to comply with CITC's policies and procedures on participant privacy may result in disciplinary action up to and including termination of employment.
• Employee may access protected health information and other participant information only on a need to know basis with others who have job responsibilities related to treatment, payment, or other CITC operations.
• Employee is encouraged and expected to report, without the threat of retaliation, any concerns regarding CITC's policies and procedures on participant privacy and any observed practices in violation of the policy to the designated Privacy Officer.

Minimum Qualifications:

Case Manager, I

• Associate's degree in psychology, sociology, human services, social work or other social services degree. Relevant experience may be substituted on a year for year basis.
• Alaska Chemical Dependency Counselor Certification I (CDC I) through the Alaska Commission of Behavioral Health Certification at time of hire or the demonstrated ability to obtain within 24 months of hire date.

Case Manager, II

• Associate's degree in psychology, sociology, human services, social work or other social service degree. Relevant experience may be substituted on a year for year basis.
• Alaska Chemical Dependency Counselor Certification II (CDC II).
• Two (2) years of experience providing case management services.
• Working knowledge of ASAM and DSM-5 diagnostic tools.

Minimum Qualifications for All Levels:

• Working knowledge of Motivational Interviewing and trauma-Informed Care.
• Experience implanting and facilitating case management best practices in the substance use field.
• Experience facilitating substance use treatment groups and individual sessions.
• Obtain Qualified Addiction Professional (QAP) approval within (3) three years of the Provisional Provider Application approval date with the State of Alaska.
• Ability to obtain a National Provider Identifier (NPI) number.
• Ability to obtain a Medicaid Identification number.
• CPR certified or ability to be certified within 30 days of hire. (Residential only)
• Continued employment is contingent upon the completion of a satisfactory state and federal background check.
• Valid Alaska driver's license and be insurable under CITC's automotive insurance, which requires a driver to be at least 21 years of age and have had a driver's license for at least three years

Preferred Qualifications for All Levels:

• Bachelor's degree in social work, Psychology, Human Services, Counseling, or Substance Abuse/Addiction Counseling.
• Relevant work experience in both residential and outpatient substance use and/or behavioral health treatment settings.

Physical Requirements:

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. May require prolonged periods of sitting at a desk and consistently working on a computer with repetitive motion.

• The need to occasionally move around the office to access filing cabinets, storage cabinets, and office equipment may be necessary.
• Must be able to lift up to 25 pounds occasionally by utilizing proper lifting techniques and working in a safe manner.
