General Functions:
As an integral part of the multidisciplinary treatment team, the Case Manager plays a key role in supporting participants throughout their recovery journey. Case Managers deliver comprehensive case management services across Recovery Services' Continuum of Care, including both outpatient and residential programs. They link participants to appropriate services, facilitate skill-building in both community and office settings, coordinate care, and provide psychoeducation-all guided by each participant's individualized treatment plan. Using a strengths-based, culturally responsive, and trauma-informed approach, Case Managers empower participants to work toward their treatment goals and prepare for long-term success beyond treatment. They actively engage clients, build meaningful rapport, and are skilled in crisis intervention, life skills training, and group facilitation.
